Coordinated Health Case Study

It can be hard to find the right doctors, understand the treatment options, make appointments and be sure that information goes to the right places. St. Joseph’s Breast Care and Surgery is dedicated to ensuring our patients coordinated access to care and to being a compassionate caregiver throughout our patients’ journey. The configuration of a referral system will vary by setting and range in complexity from a more linear referral system to a more complex referral system that makes referrals across services within the same facility and across levels of care. Data quality mechanisms with norms and protocols and ongoing training and supervision on the use of the referral system should be in place with standard referral indicators to ensure system performance and appropriate use. I was part of a 10-hospital collaborative project led by the Children’s Hospital Association that sought to change the way children’s hospitals engage families around the coordination of their children’s care, especially children with complex medical needs. The Children’s Hospital Association recently released results showing that CARE significantly reduced hospital days and emergency department discharges among 8,000 children who participated in the CARE project. That translated into a 2.6 percent reduction in Medicaid spending in the first full year of operation.

One was to reduce the use of emergency departments for routine care by establishing a primary care provider for each patient and helping the patient make and keep appointments. A second was to help patients comply with treatment recommendations — such as taking medications, making follow-up appointments, and eating well — to keep chronic conditions from worsening to the point where hospitalizations were needed. Care managers began by assessing each individual’s health care and social service needs in order to develop individual care plans. A patient whose care needs seemed to be well managed received limited care management, while one who was struggling with multiple chronic conditions received more intensive care management.

A positive change in score indicates a more satisfaction among caregivers.
